This is the same information the league uses to place you on a team — fill it in once and the webmaster will assign you to a night and squad. Once you're on a team, you'll see your schedule and scores right here.

This creates your player login and puts you in the queue to be placed on a team. It doesn't include the liability waiver — you'll still need to sign that at the official waiver page before your first game.
